Library Card Pre-Registration
Don’t have a library card? You can get a temporary card that will give you online access to eBooks, movies, music and more. After you complete the application, a staff member will contact you within 24 hours to provide you with a library card number and to confirm your PIN. This card will be valid until July 4th, 2020.
Please fill out this form to have your new library card created. When we reopen, you will need to come to the library to pick up your new card. You will need to provide a photo ID and an additional document that shows proof of your name and River Forest address. You will be emailed when your card is ready for pickup. If the patron is under the age of 14, a parent or legal guardian must be present to sign the paper application at time of pickup. *Please note that this application form is for first time cardholers only. For renewals, or student cards (living on campus at Concordia or Dominican Universities) please stop by the library and we will be happy to help you.
Statement of Responsibility for Applicant: I agree to be responsible for this card and all materials checked out on this card until it is reported lost or missing. I am further responsible for any changes that may result from late return, loss, or damage of materials borrowed. I agree to comply with Library and System regulations. By completing this form, I am agreeing to the above mentioned guidelines.
